Medication in School
Where a child needs prescription medication in school, we must have a completed form with all the details, giving us permission to administer the mediation. The most common forms of prescription medication required in school are items like inhalers, epipens and eczema cream.
Antibiotics that are required three times a day, should be taken at home. We will only accept antibiotics in school that have to be administered four times a day. These will need to be signed in and signed out.
What you need to do:
- Fill in the form below.
- Bring the medication into school in it's original box with the prescription label on
- We will then check it against the form, have it signed off by the Vice-Principal and store it for your child's use.
For any non-prescription items such as hand cream, you will need to complete the non=prescription medication form.
